Media release forms are typically sent to families at the start of the school year or during enrollment. This is an “opt-out” form. Students are assumed to be opted-in unless this form is completed by the family member.
This form can be adapted and changed based on school needs.
A Note on Signatures
Family members are authenticated with their own unique email addresses and passwords. The email is captured on form submission. This suffices as a digital signature for many schools.
